Coach's Welcome Packet -

If you are a coach and need information about coaching clinics, practice
guidelines, team placement guidelines, and concession stand, and other important
information, then this is the form for you!
If you are a coach, assistant coach, a team mom, or ANYONE who is
volunteering their time for a team, then you must fill out and submit this
form. This is an FYSA requirement. If you work with a team in any capacity,
you must submit mail this form to Ann Withkowski, Citrus United, PO Box 146,
Inverness, 34451. Once this form is submitted, it will be sent to the State for
a background check.
