COACHES NEED TO BRING

3 Copies of your FYSA printed roster (registrar will keep one copy)

All players must be verified, if not we will need a copy of birth certificate.
No players will play if added after Jan 1 2012.

Player passes and coaches passes must be laminated (no pass no play)
Coaches must be on roster, only 3 coaches are allowed on side line, all must have passes.
Show medical forms on all players, they must be notarized.

ROSTER MUST BE STAMPED BEFORE THEY PLAY 1ST GAME.

IF THE COACHES ARE NOT ABLE TO MAKE MEETING THEY MUST BE AT FIELD 1 HR BEFORE 1ST GAME TO GET CHECKED IN.
